The official Certification Handbook is coming in Q2 2026.

Eligibility Pathways

Certification is available to nurses actively practicing in hemostasis and thrombosis care across the United States and its territories. Choose your pathway based on your current licensure and scope of practice.

HT-CN

HEMOSTASIS AND THROMBOSIS CERTIFIED NURSE

Licensure: Current, full, unrestricted RN license.

Clinical Hours: At least 2,000 specialty practice hours within the last 3 years.

Education: 30 hours of HT-specific CE within the last 3 years.

Engagement: 20 professional activity points earned within the last 5 years.

HT-ACN

HEMOSTASIS AND THROMBOSIS ADVANCED CERTIFIED NURSE

Licensure: Unrestricted RN license + current APRN authorization. Graduate degree required.

Clinical Hours: At least 2,000 specialty practice hours within the last 3 years.

Education: 30 hours of HT-specific CE + 15 pharmacology hours within the last 3 years.

Engagement: 35 professional activity points earned within the last 5 years.

In April 2024, we distributed a comprehensive survey to hemostasis and thrombosis nurses at every Hemophilia Treatment Center across all regions. The response was overwhelming, underscoring a critical and unmet need for professional recognition and certification in our specialty.

The Portfolio Model

The Hemostasis and Thrombosis Certification validates clinical mastery through a comprehensive professional portfolio.

Candidates submit a detailed exemplar organized under four domains.

FEES & TIMELINE
  • The certification fee is $499 (non-refundable).

  • Based on legal guidance,  340B program funds may be used to cover the cost of this certification. We encourage candidates to speak with their HTC leadership to explore this option. 

    TIP:  Speak with your HTC leadership or administrator about 340B funding eligibility before submitting your application - this may cover your certification fee entirely.

  • Results and written feedback are provided within 60 days of your review cycle closing.

  • Certification is valid for 5 year. Recertification rquires continued licensure, continuing education, and professional activities. 

    Legacy Certifications: Individuals with an active certification from a previous body can transition through a bridge process (currently in development). If your previous certification has expires, you must apply as a new candidate.

THE PORTFOLIO & EXEMPLAR
  • The exemplar is a structured narrative demonstrating your specialty expertise, reasoning, and professional impact. It must be organized by the four domains, reflect evidence-based practice, and may include relevant de-identified objective data.

    • Current RN license (and APRN documentation if applying for the HT-ACN)

    • HT-focused CV (template provided)

    • Continuing education record (template provided)

    • Professional activities form (template provided)

    • Portfolio exemplar

    • Application form and fee

PRACTICE HOURS & EXPERIENCE
  • Hours must be specific to hemostasis and thrombosis care. General nursing practice does not count. Hours may include direct patient care or relevant non-direct care roles, provided the work requires HT knowledge and clinical judgement. 

    For HT-ACN: Hours must reflect advanced practice responsibilities such as complex clinical decision-making, prescribing/managing specialty therapies, consultation, or leadership in care planning.

    Eligible Settings: Hemophilia Treatment Centers, specialty hematology clinics, inpatient units, perioperative settings, emergency departments, infusion programs, clinical trials, and home care.

  • CE must be directly relevant to hemostasis and thrombosis nursing. For HT-ACN candidates, the required 15 hours of pharmacology CE can be general and does not need to be HT-specific.

PROFESSIONAL ACTIVITIES
  • These points recognize contributions beyond clinical hours and CE. They reflect specialty-focused professional engagement, including: presentations, posters, publications, white papers, manuscript review, quality improvement work, protocol development, committee service, leadership roles, mentorship, or precepting. A detailed list of professional activities willb e in the certificate handbook.

REVIEW PROCESS & APPEALS
  • Portfolios are reviewed by two qualified subject matter experts using a standardized rubric - a double-blind peer review process. If the two reviewers disagree on an outcome, a third independent reviewer makes the final determination.

  • Yes. Approximately 10% of candidates will be randomly selected and asked to provide primary-source documentation, such as CE certificates and employer hourss verification, to confirm the accuracy of submitted materials.

  • Candidates must achieve at least "Proficient" or "Meets Standard" in every required area of the rubric. A stronger score in one area cannot compensate for a failing score in another.

    If your portfolio does not meet the standard, the result is communiateed as "Not Yet Met". Candidates receive written feedback and may revise and resubmit after a remediation period.

  • Yes, but appeals are strictly limited. You may appeal within 30 calendar days of notification based only on:

    • Documented procedural error

    • Administrative irregularity

    • Misapplication of published policy

    Appeals are not granted simply for a disagreement with your reviewer's judgement.
